Application Submission

Fee, self-declaration, self-assessment, hospital policies, NOCs, address proof, director IDs & required documents.

Step 02

Review of application

NABH officials review the submission and send an acknowledgment letter with a unique reference number.

Step 03

Pre-assessment by committee

A Coordinating Inspector / evaluation crew verifies readiness, document authenticity and staff clarity.

Step 04

Final assessment

Once corrections are made, an NABH representative reviews all functions & services. Certificate is valid for 3 years.

Step 05

Reassessment & surveillance

Apply for reassessment at least 6 months before validity expires to keep your accreditation active.
